Sacramental Record Requests

If you were baptized, confirmed, married, or received your First Holy Communion at St. Susanna, we are happy to supply a copy of your sacramental records.  Sacramental records should be requested for religious purposes only, such as registration for religious education or marriage preparation.  To help us retrieve your records, please complete the Sacramental Record Release Request Form. 

Please note:  We do not have copies of the original certificates - only records of sacraments.
