Holy Communion for the Homebound

If you or your loved one are constrained for medical reasons from attending Mass and would like to have a Eucharistic Minister bring Holy Communion on a scheduled basis in your home (including assisted living, senior living, nursing home), please click on the link below and fill out the form.  All information entered on the form will be kept strictly confidential by the parish secretary, priest, and our homebound coordinator, who will contact you to arrange a time.  ​
